Level Two Adult Weight Management Service

Introduction

The Level Two Adult Weight Management Service contributes to the All Wales Weight Management Pathway 2021. This aims to support Powys residents who are overweight and wish to make diet and lifestyle changes to achieve a better quality of life through improved health, wellbeing and weight loss.

 

What does the Level Two Weight Management Programme involve?

It is made up of multi-component interventions addressing diet, physical activity, and behaviour change, underpinned by behavioural science.

 

Who is the Level Two Weight Management Programme For?

The programme is open to adults living in Powys, or who have a Powys GP (General Practitioner), who have either:

  • A Body Mass Index (BMI) over 25kg / m2 and are living with co-morbidities

OR

  • A BMI over 30 kg / m2 and are living without co-morbidities

The programme is not suitable for individuals who:

  • Have an active eating disorder(s)
  • Are pregnant
  • Have issues with alcohol or drug misuse
  • Have a psychotic illness(s)
  • Have uncontrolled Type One Diabetes
  • Have recently had or are expecting bariatric surgery

These individuals will be assessed by the Dietetic Team on referral.

 

What will be expected of me if I join the programme?

Motivation to achieve a healthy weight and engagement with courses. Interventions will be delivered in groups, either virtually or face-to-face, and patients should be willing to commit to weekly sessions. Group times are offered flexibly, where possible including evenings.

The length of time within the service will be tailored to individuals, but usually lasts a minimum of 12 weeks. At the end of this period, progress and patient satisfaction will be evaluated for service development.

 

What can I expect from the Level Two Adult Weight Management Programme?

Specialist Dietetic Assistant Practitioners will assess you and direct you to a suite of courses which help motivate and support you in making healthier choices. These include:

  • Dietary education (‘Foodwise for Life’, or ‘Slimming World’)
  • Developing cooking skills (‘Get Cooking’)
  • Increasing exercise (‘National Exercise Referral Scheme (NERS)’)
  • Or more tailored support as required.

Information for Healthcare Professionals
  • Patients who wish to lose weight, but aren’t suitable for the Level Two Service, can be referred into clinic. Please use the form on the PTHB intranet
  • BMI inclusion criteria are 2.5 kg / m2 lower for Afro Caribbean and Asian groups due to higher risk factors
  • Access to Level Two Services is strongly encouraged before referral to Level Three Services is considered
  • Programmes are run across PTHB in health board buildings and community venues or using virtual platforms. Special access arrangements will be accommodated where possible
  • Please refer under-18s to the Paediatric Dietetics service
