Step 1: Check the Information for Healthcare Professionals Page
- Before completing a referral, health professionals should check the Information for Healthcare Professionals pages to make sure the referral is appropriate for the service.
Step 2: Use WCCG
- GPs and primary care colleagues are asked to send referrals via WCCG.
- This is the fastest way for the service to triage and review referrals.
Step 3: If WCCG Access Is Not Available
Step 4: What to Include in the Referral
Whether using WCCG or a letter, the referral should include:
- The main reason for the referral.
- Relevant information to help with triage, such as:
- Recent blood test results.
- Details of current medication.
- Any other useful clinical information, including clinic letters that would not appear on Welsh Clinical Portal.
