How to refer

Step 1: Check the Information for Healthcare Professionals Page

• Before completing a referral, health professionals should check the Information for Healthcare Professionals pages to make sure the referral is appropriate for the service.

Step 2: Use WCCG

• GPs and primary care colleagues are asked to send referrals via WCCG.
• This is the fastest way for the service to triage and review referrals.

Step 3: If WCCG Access Is Not Available

• If WCCG cannot be used, referrals should be sent by letter to: PLWS@wales.nhs.uk

Step 4: What to Include in the Referral
Whether using WCCG or a letter, the referral should include:

• The main reason for the referral.
• Relevant information to help with triage, such as:
  • Recent blood test results.
  • Details of current medication.
  • Any other useful clinical information, including clinic letters that would not appear on Welsh Clinical Portal.
