PLWS Referral Exclusion Criteria

Referral will be declined if:

• Person’s health condition does not fall within our remit (link to conditions supported page)
• Person has an unmanaged drug or alcohol problem
• Person has already been referred, or is being treated, elsewhere for the same condition
• Person is awaiting investigations for same presentation
• Person has an untreated serious mental health problem
• Person is under 16

We will not accept weight management referrals for pregnant individuals.

For these cases, please seek advice from Powys.Dietetics@wales.nhs.uk instead.

Once you are satisfied that the person is eligible for referral to our Service, please ensure

you include all the results for the required Blood Tests when submitting the referral on WCCG.
