After receiving a referral, the Powys Living Well Service team reviews the information provided. If any details are unclear or incomplete, the team may contact the referrer for clarification before accepting the referral. Once accepted, the individual is sent a welcome booklet explaining the next steps.
Step 2: Initial Engagement
The individual is invited to read the welcome information and complete an online “Information About You” questionnaire. This helps the team understand their needs and preferences.
Step 3: Appointment Scheduling
When the completed questionnaire is received, the team reviews the responses and confirms the date and time of the first consultation. Information about the available support options is also provided.
Step 4: Preparation for Consultation
The individual is encouraged to read through the options, reflect on what matters most to them, and consider which areas they would like to explore.
Step 5: First Consultation
During the initial consultation, the team discusses the options identified by the individual and agrees on a personalised plan. After this appointment, the referrer receives a summary letter outlining the discussion and agreed next steps.
