Modules

1. Building Better Health
Introduces practical ways to live well with a long-term condition. Participants reflect on healthy lifestyle choices, explore self-management tools such as mindfulness, and learn strategies to support wellbeing day-to-day.

2. Healthy Choices
Focuses on making healthy choices around food and drink. Participants explore what healthy eating means for them, reflect on current habits, and practise mindful eating as a tool for long-term wellbeing.

3. Keeping Active
Explores how to stay active while managing a long-term condition. Participants learn the benefits of physical activity, how to avoid the “boom and bust” cycle, and practise mindful movement to support energy and wellbeing.

4. Managing Your Mood
Helps participants understand and manage mood. The module looks at factors that affect how we feel, explores quick fixes and longer-term strategies, and introduces mindfulness as a tool for emotional wellbeing.

5. Breathlessness
Provides guidance on managing breathlessness. Participants learn about its causes, practise techniques such as positions of ease and breathing control, and discover how planning ahead can reduce anxiety and improve confidence.

6. Staying Connected
Highlights the importance of social connection while living with a long-term condition. Participants explore how communication supports wellbeing, practise useful strategies, and learn about local resources.
