Rehabilitation Therapy Support Worker

Mo joined the Powys Living Well Service as a Rehabilitation Therapy Support Worker in February 2026. She works closely with the multidisciplinary team to deliver person‑centred support that helps individuals build independence and develop effective self‑management skills to maintain their health and wellbeing.

Her role includes supporting the delivery of rehabilitation programmes, monitoring progress against agreed goals and values, and contributing to ongoing assessment and review to ensure care is safe, effective, and focused on meaningful outcomes.

Before joining the service, Mo worked in a Special Educational Needs (SEN) school, supporting children with a wide range of additional needs. This experience gave her a strong foundation in providing tailored support, communicating clearly, and working collaboratively within multidisciplinary environments.

Mo is committed to delivering high‑quality, person‑centred care. She is passionate about helping people build confidence, maximise independence, and achieve outcomes that are important to them.
