Health Coaching

At Powys Living Well Service, our aim is to help you live the life you want, even if you have a long-term health condition. Health coaching is one practical way we can support you to do that.

Health coaching gives you time to think about what matters to you and to choose an objective to work towards. You will use the resources you already have to help you move in that direction, and you will agree small, manageable steps to take next. Progress can take time, but using the health coaching process can improve your chances of reaching your objective.

Stage 1: Learn about the coaching process

You will start by attending one of our online group coaching sessions (around 90 minutes). You do not need to book a place on this session. Just follow the link below at the time of the session you want to join.

During the session, we will introduce the coaching model and support you to think about the objective you would like to set. We will also explain a simple process you can use to increase your chances of successfully reaching your objective.

Group coaching sessions are held once a month. They are available to anyone who has already attended an initial consultation with a member of our team. You do not need to book, just use the link below on the date of your preferred session. We recommend joining a few minutes before the session starts.

Stage 2: One-to-one coaching (Optional)

After you have attended a group coaching session, you can request one-to-one coaching sessions with a member of the team to help you keep your momentum going. To request a one-to-one coaching session, please contact the service after the group session by email to PLWS@wales.nhs.uk at any time, or by calling 01874 442910 (between 9am and 12 noon, Mondays to Fridays).
