Specialist Palliative Care

Who do we support?

Your Powys Specialist Palliative Care team provide support and advice for patients and their families with a life limiting illness whose symptoms are complex to manage.

Opening hours

We are a non-emergency service, available Monday to Friday, 08:30-16:30. On weekends and bank holidays please contact your out of hours GP service.

How to access the service

The Specialist Palliative Care Service accept referrals from GP’s, inpatient wards, hospital specialist teams and consultants.

What to expect

Working with hospital and community services in partnership with you, your family or carer, we can offer specialist support and advice to patients and their families at home, in care home settings and in Powys community hospitals including:

  • Advice on control of complex symptoms e.g. pain, sickness, poor bowel management and appetite.
  • Complex Psychological, and spiritual support including pre bereavement.
  • Information and practical advice eg finances, local support services
  • A resource of information, specialist advice and support on palliative care for other health professionals.
  • Initial post bereavement support and signposting to those who have required specialist palliative care input.

Our teams

South Powys, covering the GP practices in the following areas- Hay on Wye, Talgarth, Crickhowell, Brecon and Ystradgynlais.

Mid Powys- covering the GP practices in Builth wells, Llandrindod, Rhayader, Knighton and Presteigne.

North Powys- covering the GP practices in Llanidloes, Newtown, Welshpool, Machynlleth, Montgomery, Welshpool, Llanfyllin, Llanfair Caereinion.
