Your Local Memory Assessment Service

Who do Memory Assessment services support?

The Memory Assessment Service (MAS) support people who may be experiencing memory problems or changes in their ability to function in their activities of daily living, at a level that they would normally expect to do so.

Consideration is given to all ages and the service offers a multi-disciplinary assessment of memory, which may initially be completed at a clinic appointment or if appropriate may be offered in the patient’s home.

 

Opening hours

The Memory Assessment Service operate core hours between 9am – 17:00 Monday to Friday

 

How to access the service

To access these services please contact your GP they will make a referral to the service.

 

What to expect

The MAS teams provide holistic assessments, that are effective in providing the best outcomes for the person being assessed, diagnosis and treatment where appropriate. Post diagnostic support is available following a dementia diagnosis.

Physical health concerns must be addressed prior to attending the service, and this will be completed by the GP prior to referral.

Follow up visit for ongoing assessment will be arranged if necessary.
